If you did not turn in your CME evaluation form at the conference, please download the form below to claim only the credits you qualify for, and email it directly to the CME provider, NACE, at:
 
 
A CME certificate will be emailed to you within 4 weeks. 
 
 
AAD Credits
If you are claiming AAD credit, please go online to the AAD website www.aad.org and use the Atlantic Derm code #82100 to claim your AAD credit.
